We try our best to stay on time as we know your time is valuable. Emergencies sometimes detain us at the hospital and some patients require more time than anticipated. We appreciate your understanding and realization that you will also get dedicated time and attention so you may leave our office with a better understanding of your condition. 

If you need to cancel or reschedule you appointment, please let us know as soon as possible. This courtesy allows other patients to be seen more conveniently.

Please carefully read the following information, so you will be well-prepared for your first visit. 

• All minors must be accompanied by a parent or guardian. If a parent or guardian is not available, please contact our office for instructions regarding consent to bring the minor to an appointment.
• Remember to bring a photo I.D., current insurance card(s), and your copay/deductible payment in the form of cash or card. We do not accept checks.
• Please bring all test results including labs and diagnostic imaging pertaining to the reason for you visit. If you do not have these documents, please make sure they are faxed to our office prior to your appointment.
